Medical Surgical Specialties
Clinical Services: Inpatient medical/telemetry/isolation unit providing care to normal volunteers, protocols involving a requirement for isolation, including the use of live virus vaccines or challenge studies and those employing intensive pharmacokinetics or other types of intense nursing interactions. The unit serves the medical needs of laboratory workers at federal BSL-3/4 research facilities who have sustained a potential occupational exposure to an infectious pathogen and require close clinical observation and possible treatment under strict isolation precautions. This unit has the ability to respond to global health emergencies such as Ebola and SARS coV-2.
Age of Patient Population: Adults over 18 years of age.
Clinical Facilities: 1 isolation room and 3 Private rooms for a total of 4 beds.
Research Focus: NIAID, CCMD and NHLBI SARS-CoV2 protocols; routinely NIAID protocols to include but not limited to live vaccine administration.
Admitting Institutes: NIAID, NHLBI and CCMD; other institutes with research protocols deemed appropriate for this specialized inpatient facility upon review by unit Medical Director.
Special Environmental Support: The entire unit is negative air flow with the exception of the nursing station and the day room. The unit is equipped for cardiac monitoring; two monitors in the hallway and a central monitor at the nurse’s station. One room has an anteroom designed to care for individuals with an occupational exposure to a Bio-Safety Lab (BSL) 3 or 4 pathogen. The unit has an on-site autoclave, bio sealer, hand sealer, and a bed isolation unit for use with patients on special respiratory isolation (SRI).
Special Patient Care Needs: Isolation, cardiac monitoring, Cyraphone interpretation services; intensive patient education related to protocols, isolation, and illness management, ACLS and ONS Chemotherapy providers.

Clinical Services: Inpatient nursing care of patients with various endocrine, infectious, autoimmune, metabolic, pulmonary, gastroenterological, hepatic, genetic, and lipid disorders. Serial and provocative blood testing, intravenous catheter care, post-endoscopic procedure care, Point of Care Testing (POCT) blood glucose and specific gravity testing, metabolic testing, skin and wound care, I-131 and Lutathera radiation treatments, liver and skin biopsy, pain management, immunotherapy treatments, and cardiac telemetry monitoring.
Age of Patient Population: Range from 18 years through the life span.
Clinical Facilities: 22 inpatient beds, 2 treatment rooms, 2 lead lined radiation treatment rooms, and 1 negative pressure room.
Research Focus: Endocrine, infectious, autoimmune, metabolic, pulmonary, gastroenterological, hepatic, lipid and genetic disorders. Studies focusing on disorders of calcium/bones, pituitary, gonads, thyroid, pancreas, and adrenal glands, lung disease, lipid disorders, gastrointestinal disorders, chronic hepatitis B & C, Sickle Cell disease, and inborn errors of metabolism.

Special Environmental Support: Radiation isolation rooms and a negative pressure room is available for populations requiring this support.
Special Patient Education Needs: Teaching for radiation safety and Adrenal Insufficiency. Patient education related to diagnosis, serial blood testing, intravenous catheter care, endoscopic procedures, blood glucose and specific gravity testing, metabolic testing, skin and wound care, post-operative surgical care, and pain management.
Specialized Competencies: Cardiac Telemetry Monitoring, Blood Products, Chest Tube Management, Tracheostomy, Controlled Substance Infusion (IV, PCA, Epidural), Intravenous Therapy, Oxygen Delivery Devices, POCT - Bedside Blood Glucose Monitoring (BBGM) and Specific Gravity, Radionuclide Therapy, Non-Oncologic Chemo-Biotherapy, and Safe Handling of Hazardous Drugs.
Clinical Services: This is an Inpatient research focused medical/surgical/telemetry unit which provides care to newly diagnosed, acutely, chronically ill, and end of life patients with infectious disease, immune disorders, respiratory, rheumatologic, cardiac and sickle cell disease. Care is provided to patients needing cardiac monitoring and cardiology consult services. Negative pressure rooms are available, for patients from all institutes, needing AFB isolation plus a designated section for the care and monitoring of all non-critical hospitalized patients who need highly specialized enhanced isolation. The unit also has the capabilities to support CC patients suspected of or positive for emerging infectious diseases.
Age of Patient Population: Adults over 18 years of age.
Clinical Facilities: 20 private rooms
Research Focus: NIAID, CCMD and NHLBI SARS-CoV2 protocols; Phase I, II, and III protocols of NIAID, NIAMS, NIEHS, NHLBI, NEI, NCI and NIDDK, as well as an in-depth clinical assessment of patients on natural history protocols.
Admitting Institutes: NIAID, NHLBI, CCMD all institutes who have patients requiring specialized enhanced isolation. NIAID, NIAMS, NIEHS, NHLBI, NIDDK, NCI and surgical specialties such as NEI and the dental clinic.
Special Environmental Support: Cardiac monitoring. Four isolation/negative air flow rooms with vestibules; three to four rooms and one nurse station designated for a highly specialized isolation area used for the monitoring and care of all non-critical hospitalized CC patients with specialized isolation needs.
Special Patient Care Needs: Cyraphone interpretation services. Patient education is provided regarding coronary and peripheral artery disease, hypertension, congestive heart failure, venous access device management, infectious diseases, isolation, treatment of iron overload, treatment of pulmonary hypertension, home use of infusion pumps for Desferol and Remodulin, immunosuppressive medications, gene therapy and a heart healthy lifestyle. Patients requiring High Flow Oxygen Device.
Specialized Competencies: Clinical Nursing care supporting patients participating in research studies at the clinical center: Blood Products, Cardiac Monitoring, Chemotherapy/Biotherapy, Chest Tube Management, Controlled Substance Infusion, Intravenous Therapy, Oxygen Delivery Devices, Point of Care Testing (POCT) Bedside Blood Glucose Monitoring (BBGM), Pre and Post Invasive Cardiac Procedure, Safe Handling of Hazardous Drugs, Skin and Wound Care, Special Respiratory Isolation, Enhanced Respiratory Isolation, Enhanced Contact Isolation, Telephone Triage, Temporary Pacemaker, Tracheostomy Care, ACLS and ONS chemotherapy certification also required.
Clinical Services: Multi-institute diagnostic and therapeutic day hospital serving patients with endocrine, metabolic, allergic, immunologic, infectious disease, ophthalmologic and neurological disorders, gynecological, rheumatologic, and nephrology consult services. Clinical Research Nurses assess, plan, and coordinate patient care. CRNs perform and monitor tests, assist with procedures, and administer treatments amongst providing many other services.
Age of Patient Population: 18 years of age or older

Dialysis: ACLS and cardiac monitoring.
Clinical Services: Multi-institute in-patient and out-patient setting for supporting clinical protocols that focuses on the study of metabolic processes and nutritional diseases, such as obesity.
Age of Patient Population: Patients are 18 years of age through the lifespan. Pediatric patients, at least 8 years of age, requiring specific metabolic needs, are cared for in collaboration with members of the metabolic program and pediatric program teams.
Clinical Facilities
- 5SW-N: An eight-bed inpatient unit providing 24-hour nursing care to patients participating in metabolic studies. The area supports an exercise testing laboratory, a body scanner, experimental dining area, metabolic kitchen, enlarged shower room, and an exam room designated for outpatient studies.
- 7SW-S: An Inpatient unit providing 23 hours of nursing care to patients undergoing metabolic testing. The area supports three whole-room indirect calorimetry suites with telemetry monitoring capability, three metabolic testing procedure chairs, a body composition room, and an enlarged shower room.
Research Focus: The initial focus of the work is endocrine and metabolic studies related to obesity. Such studies include investigations of the effects of weight gain and weight loss, the physiology and pathophysiology of energy metabolism, and studies related to the co-morbid conditions associated with obesity, including type 2 diabetes mellitus and its complications. Other disorders of metabolism requiring the specialized equipment of the metabolic unit and metabolic suite are eligible for study. Patients are evaluated in both inpatient and outpatient settings. Patients include non-obese and obese research volunteers. All study patients/volunteers considered for study are 450 pounds or less, ambulatory, and able to provide self-care.
Admitting Institutes: All NIH Institutes and Centers with protocols commensurate with research focus.
Special Environmental Support: Expanded capacity supplies, equipment and furniture to accommodate weights up to 450 pounds, YSI machines, treadmills, large scales and wall-mounted stadiometers, upright and recumbent bikes, Bod Pod, DEXA scanner. The unit has three metabolic suites, four metabolic carts, a metabolic kitchen, experimental dining area, and three specialized vending machines.
Clinical Services: Dental consultation service, treatment for dental defects and extractions, conscious sedation extractions of third molar, evaluation of craniofacial defects, oral and lip biopsies, salivary gland harvesting, study of symptoms of Sjőgren’s Syndrome. Usage of the stereo photogrammetry machine, photobiomodulation THOR device, taking digital panographic x-rays, digital cephalometric x-rays, and performing skin biopsies.
Age of Patient Population: Early childhood through adults.
Clinical Facilities: 2 Dental Surgical procedure rooms, 1 Dental Recovery room, 5 Dental Operatory rooms, 1 medical exam room, 1 X-ray room, 1 cone beam CT scanner room, Stereo photogrammetry machine, 1 NIDCR Clinic Lab.
Research Focus: 2D and 3D imaging proteomics, salivary gland dysfunction & Sjőgren’s Syndrome, oral phenotyping of genetic diseases, gene transfer and genetic periodontal diseases, craniofacial anomalies and developmental growth variant, study of microbiome and study of patients with Graft-versus-host-disease (GVHD), evaluation and treatment of bone and mineral disorders, periodontal manifestation of Leukocyte Adhesion Deficiency, McCune Albright syndrome and treatment of tumor induced osteomalacia.

Special Patient Care Needs: Immunocompromised research participants who have gum disease and plaque buildup are taught how to use the End brush and Chlorhexidine rinse. Alcohol Dependency Study research participants are given recommendations for obtaining optimal Dental Health. Pre and Post Stem Cell and Bone Marrow Transplant participants are evaluated and prescribed treatment. Research participants with Behavioral Health disorders are given recommendations on Dental Care. GVHD research participants are evaluated for oral lesions and overall oral health as well as their oral hygiene. Treatments of this population consist of prescribing supplemental fluoride treatments, Triamcinolone injections and/or antibiotics or steroidal mouth rinses and progress is monitored. Sjőgren’s Syndrome research participants are given gene therapy for radiation induced xerostomia to increase salivary flow.

- Neurology/Neurosurgery:Deep Brain Stimulation Activation (DBS), Sutures and Staples removal, ON/OFF Evaluation of Parkinson’s Disease patients, Botox administration, Skin punch biopsies, Smell tests, Coordination tests and Cognitive functions tests, Optical Coherence Tomography (OCT).
- Audiology: Vestibular Head Impulse Testing (VHIT), Complex Auditory Brain Stem Response (CABR), Audiogram.

Special Patient Care Needs: Specific guidelines for patients who are immunocompromised and /or neutropenic. Treatment is readily available for febrile neutropenia. A designated treatment area to meet the needs of more complex patients requiring medications, vaccines, and electrolyte replacement. A pharmacist is available in the clinic 1 day a week to facilitate care. Use of protocol coordinators to facilitate research and care of patients based on disease etiology. Nurse-led patient education clinics for complex hematology and pulmonary patients. Flexible patient scheduling system to meet the care demands of the patient population. Utilizing consultants and the interdisciplinary team, a patient-centered care model is used with complex patients to facilitate the delivery of patient care.
Specialized Competencies: Telephone Triage, Intravenous Therapy, Safe Handling of Hazardous drugs, Medication Administration, and Stem Cell Transplant Care.
Clinical Services: Initial and Follow-up Nursing Assessments, Emerging Infections analysis, Abuse and Neglect screenings, and Falls risk analysis/prevention. Clinical Services: This clinic is a comprehensive outpatient area that performs structured bio-psychosocial assessment of alcohol and drug use problems, as well as healthy volunteers for inclusion into clinical studies of substance use disorders.
Age of Patient Population: Late adolescent to adulthood.
Clinical Facilities: 3 exam rooms (2 with beds), 1 neuropsychological interview room, 1 cognitive testing room, one group therapy room.
Research Focus: Substance use disorders related protocols.
Institutes Providing Services: NIAAA, NIDA, NIMHD
Special Patient Care Needs: A safe and secure and private environment.
Specialized Competencies: Prevention and management of aggressive behavior, suicide-self harm prevention, locked seclusion, restraint application and maintenance, positional restraint, team control and transport, seizure management, psychological ratings, and withdrawal assessment & management.
Clinical Services: All pediatric research participants that require a screening or testing in outpatient setting, with the exception of those on behavioral health protocols.
Age of Patient Population: Pediatric patients with diseases that extend into adulthood are seen in the pediatric clinic until age 30 when they are transitioned to appropriate adult outpatient clinical areas.
Clinical Facilities: This outpatient clinic has 14 exam rooms, two triage rooms, one isolation/exam room, one procedure room, three interview rooms, one playroom and two testing room for pediatric research participants requiring care.
Research Focus: All NIH studies involving pediatric research participants that do not require inpatient admission or Day Hospital level of care.
Admission Privileges: All NIH Institutes with IRB approved protocols.

Clinical Services: The pediatric outpatient day hospital provides care for pediatric patients that do not require overnight admission.

Clinical Facilities: The Day Hospital has six rooms, which includes seven beds, six treatment chairs, one vital signs room, and a procedure room for pediatric research participants requiring care. The DH can accommodate for 1 to 2 patients on isolation precautions.

Special Environmental Support: Behavioral Health Milieu with psychosocial treatment modalities.
Clinical Services Provided: The alcohol and addictions unit conducts interdisciplinary assessments and treatment of alcohol and drug abuse addiction problems. This includes medical and psychiatric co-morbidities and intensive management of suicidality and aggression. Clinical monitoring includes severity and dependency measures, family and psychosocial factors, and personality measures. A structured four-week treatment program provides Family Education, Alcoholics Anonymous® and Smart Recovery® programs, and multiple Rehabilitation Medicine group and individual therapies.
Special Patient Care Needs: A safe environment is maintained and complies with federal confidentiality laws regarding alcohol treatment. IRB approved use of certificate of confidentiality.
Specialized Competencies: All nurses are skilled in addictions and psychiatric nursing and have demonstrated competence in Behavioral Health nursing prevention and management of aggressive behavior, suicide-self harm prevention, locked seclusion, mechanical restraints, seizure management, intravenous therapy and venipuncture, and withdrawal management. Additionally, all nursing staff receive certification in the Crisis Prevention Institute training for the prevention of violence/aggressive behavior. Specialized competencies of nursing and clinical research staff expertise include PET studies, FMRI studies, infusion studies, serial testing, drug trials, lumbar punctures and psychological ratings scales.
Clinical Services: Includes inpatient and day hospital treatment in child and adolescent psychiatric research and care.
Age of Patient Population: Patients range from 4-17 years of age and are admitted from national and international localities.
Clinical Facilities: 1 SW is a 6-bed unit with flex capacity for patients in Day Treatment programs. The unit has four dedicated inpatient rooms with two additional rooms serving as flex beds. Flex Beds: 1 testing suite (used for overnight sleep studies/video EEG) and 1-day hospital bed.
Research Focus: Autoimmune encephalitis, sleep disorders, adolescent depression.

One procedure room is available for use on the unit that has the capability for EEG video telemetry, video monitoring, and Transcranial Magnetic Stimulation. Special Patient Care Needs: Locked unit, milieu management, and other measures to ensure safety. Observation levels are based on patient needs for safety and support and can range from 1:1 or 2:1 staff monitoring to every 30-minute checks. Social work, recreation therapy, occupational therapy, art therapy, speech and language therapy, nutritional care, and pastoral care services are provided as needed.

Interdisciplinary teams include clinical social worker support, a clinical pharmacy specialist, a recreational therapist, an occupational therapist, and a human subjects protection unit (HSPU) team. Also provided are an independent medication management program, behavioral rating scales, community living groups, and research education.
Specialized Competencies: Psychopharmacology, neuropsychological testing, psychopathology ratings, sleep studies, FMRI studies, drug trials (proof of concept studies), PET studies, infusion studies, lumbar puncture, genetics, behavioral health crisis nursing prevention, and management of aggressive behavior. Clinical Services: The operating room attends to patients enrolled on experimental protocols that require surgical interventions, diagnostic and invasive therapeutic procedures. Over 2000 surgical procedures are performed annually in the main operating room suites.

Clinical Facilities: Operating Room consists of 11 operating room suites, including 1 intraoperative MRI suite, and three rooms dedicated to laparoscopic/robotic surgery. Research Focus: All patients are involved in a clinical trial. Surgeries include but are not limited to: biopsies, laparotomies, perfusions, craniotomies, robotic surgery, and endoscopy procedures.

Oncology and Critical Care
Clinical Services: Comprehensive medical care and research support offered to patients with solid tumors, malignant and non-malignant hematologic disorders, lymphomas, leukemia’s, autoimmune diseases, immunological disorders, and those patients receiving blood and marrow transplants (e.g. bone marrow and peripheral blood stem cells from autologous and allogeneic sources). Administer per protocol regimens with chemotherapy, biotherapy, gene therapy auto transplants and targeted immunotherapies, e.g., CAR cells.

Clinical Services: Acutely ill patients requiring intensive monitoring or advanced life support are eligible for admission to the Intensive Care Unit. Intensive monitoring includes hemodynamic, respiratory and cardiac as well as complex nursing care, post- procedure observation, continuous renal hemofiltration/therapy and other interventions for life-threatening problems. Patients requiring specialized or continuous monitoring for a research protocol are also eligible for ICU admission.
Urgent ACLS- Level Transport: Responsible for the care of a patient requiring urgent transport to another facility (Suburban Hospital) under the direction of a CCMD Attending.
After Hours Triage: Management of care for the unplanned after-hours adult outpatients presenting with acute care needs. This program utilizes a designated “triage bed” in the ICU for a maximum 2-hour stay. The patient’s primary medical team is the team responsible for the care and treatment of the triage patient until a decision is made to admit or discharge the patient home.
Rapid Response Team: Attends to all inpatient Rapid Response calls which include calls from any medical team, staff, the patient, and family members.
Code Blue Team: Responds to all Code Blue emergencies within the Clinical Center and the Magnuson Building which includes patients, visitors and employees.
Age of Patient Population: Three years of age (>15kg) through late adults.
Clinical Facilities: Twelve private intensive care rooms on 3SW-S, and six private rooms on 3SW-N that serve as ICU overflow and/or specialized Isolation for critically ill patients.
Research Focus: Phase I, II, and III protocols from all institutes within the Clinical Center.
Admitting Institutes: Supports patient populations from all Institutes within the Clinical Center that require Critical Care Services. Emergency support to the state of Maryland to accept ICU or critically ill patients from area hospitals during states of emergency.
Special Environmental Support: Three negative air flow rooms with vestibules; six hepa-filtered rooms; video monitoring capability, cardiac/hemodynamic monitoring, advanced life support, bedside computers on wheels, and hemodialysis connections, Clinical Data Viewer Flowsheet, Point of Care Testing.
Pandemic Preparedness: Ability to adapt to any epidemiological environment by changing unit workflow and environment.
Special Patient Care Needs: Cyraphone/AT&T interpretation services, word/picture charts for intubated patients, preoperative teaching and tours of the ICU, family waiting area and private consultation rooms, sleeper chairs for "rooming in", open interdisciplinary rounds to include family twice a day, weekly family meetings.

Clinical Services: Provides patient centered care to oncology research patients receiving radiation therapy. The care provided includes radiation therapy treatment delivery, care coordination, patient education, medication administration, research sampling (blood, urine), symptom management, short- and long-term treatment follow up, survivorship, emotional support services.

Specialized Competencies: Telephone triage, radiation safety, administration of chemotherapy/biotherapy, intravenous therapy.

- Donor collection of whole blood and apheresis components in support of:
- Clinical Center patient transfusion needs, including collection of whole blood by phlebotomy and packed red blood cells by apheresis, and the performance of automated plasmapheresis procedures in volunteer donors.
- Collection of blood components for NIH institute (intramural) investigators, including collection of whole blood, leukapheresis, and hematopoietic progenitor cell collections, plateletpheresis, and plasmapheresis components from normal volunteers under protocol 99-CC-0168.
- Patient apheresis procedures, including:
- Procedures performed for therapeutic intent (including plasma exchange, red cell exchange, leukapheresis, and extracorporeal photopheresis)
- Procedures performed as part of clinical trials investigating the therapeutic efficacy of apheresis.
- Procedures performed to collect cellular therapy components for hematopoietic reconstitution, generally as part of collaborative hematopoietic transplantation protocols
- Procedures performed to collect components for further ex vivo manipulation, such as long-term culture, CAR-T cells, gene therapy, tumor vaccine development or targeted cell therapy.
- Procedures performed to collect components from patients for in vitro research use by institute investigators (covered by IRB-approved protocols)

Clinical Services: We provide care to Medical/Surgical oncology patients on Phase I and II research protocols. The care provided includes patient education, medication administration, research sampling (blood, urine, tissues), and oncological symptom management.
